You must comply if you are self-employed doctor, consultant, locum, independent consultant, medical professionals with private practice or receive property income, and your gross business/property income exceeds the threshold.
From 6 April 2026 the threshold is £50,000. It falls to £30,000 from April 2027 and £20,000 from April 2028. Many doctors with private fees or rental income will fall in scope for MTD for doctors.
If you are salaried only, with no self-employed income and no property business, you may not be in scope. However:
Self-employed locum doctors working via agencies or private practice are in scope.
If you receive property income above threshold then MTD applies.
Doctors in partnerships may still need to comply for their personal self-employed income streams.
Each quarter, doctors must report via MTD for doctors software:
Total professional income (consultancy, locum fees, private practice, sessional work)
Allowable practice-related expenses (rent, insurance, CPD, travel)
