Which doctors need to comply with MTD and when does it start?

You must comply if you are self-employed doctor, consultant, locum, independent consultant, medical professionals with private practice or receive property income, and your gross business/property income exceeds the threshold.

From 6 April 2026 the threshold is £50,000. It falls to £30,000 from April 2027 and £20,000 from April 2028. Many doctors with private fees or rental income will fall in scope for MTD for doctors.

Do salaried doctors, NHS consultants or doctors in partnerships need to register?

If you are salaried only, with no self-employed income and no property business, you may not be in scope. However:

  • Self-employed locum doctors working via agencies or private practice are in scope.

  • If you receive property income above threshold then MTD applies.

  • Doctors in partnerships may still need to comply for their personal self-employed income streams.

What do doctors need to submit under MTD?

Each quarter, doctors must report via MTD for doctors software:

  • Total professional income (consultancy, locum fees, private practice, sessional work)

  • Allowable practice-related expenses (rent, insurance, CPD, travel)

  • Digital records aligned to HMRC categories. You do not need to submit receipts or invoices each quarter - just totals by category. At year end you submit your final declaration including other income (dividends, interest, property

What software do doctors and medical professionals need for MTD?

You must use HMRC-recognised MTD software for doctors (or bridging software) for digital records and submissions. It must support your income streams (self-employment, property) and link to HMRC via API. Popular choices among medical professionals include cloud accounting packages with specialist support. 

Can my accountant or tax adviser handle MTD for me?

Yes. Your accountant or tax adviser can act as your agent, managing your digital bookkeeping, quarterly submissions and year-end final declaration using MTD software for doctors. However, you remain legally responsible for accuracy and timely filing.

What happens if I don’t comply with Making Tax Digital?

Failure to comply may attract penalties from HM Revenue & Customs including points-based penalties for late or missing updates. Besides compliance risk, poor digital records can lead to inaccurate tax planning and unexpected tax bills.

Does MTD cover doctors’ rental income or other income streams?

Yes. If you are a doctor with mixed income (professional fees + rental property, dividends, interest) MTD may still apply when combined self-employed + property income exceeds threshold. Each income source must be reported digitally separately.
